At the time of your appointment: We require government issued photo ID prior to all procedures for all clients, even for repeat customers (i.e. driver's license, health card, Ontario photo card, Passport, LCBO Age of Majority Card).
Must be 16 years+ (consideration extended to ages 14 - 15 *with parental consent).
